I often get calls from friends of friends or friends caring for loved ones unsure of what to do when a loved on goes into the hospital. What is the next step to get them home? Where is the best place for your loved one to be discharged to? Often times, there will be a Social Worker or Case Manager at the hospital that you might have to reach out to and my advice is always, do that the day your loved one is admitted. There is a saying in healthcare that discharge planning starts on the day of admission. The conversation must start early and often! Not sure where to start? Call the nursing station where your loved one is and ask to speak to the Social Worker. Starting the conversation ensures that you and your loved one will be involved in the decision making. Discuss goals for care with the hospital team, discuss as a family and present your wishes to the team. Most of the time, when the patient is ready for discharge, the social worker will be looking quickly as to where to transfer the patient. They want to work with you on the next steps but often times, those conversations become rushed when there is a timeline. My advice is to make contact early and stay in touch with the team. Communication is key!
